A referral from the patients' primary physician is necessary for participation in this program.

Explanation of services

The Diabetes Care Center provides multi-disciplinary diabetes care. Intensive diabetes control, such as insulin pump use, the center enables patients to live rich and full lives despite their condition.

This program is an enjoyable and effective way to help patients:

Gain confidence in their physical capacity

Understand the role of nutrition in heart health.
