Passport to Heart Health for Corporations

FOCUS OF THE PROGRAM
The purpose of the program is to identify individuals at risk for a heart attack or stroke through screening. Several major risk factors for heart disease are assessed. Educational materials are provided about how nutrition, exercise and other lifestyle factors can be modified to lower risk. In addition, counseling about stress management and the signs and symptoms of heart disease are provided.

HOW THE PROGRAM OPERATES
The service is operated as a mobile unit traveling to interested corporations. Workstations will be set up to perform screenings that include the following: Cholesterol, Glucose (sugar level to check for diabetes), and Blood Pressure. A specialized staff from NewYork-Presbyterian Hospital Preventive Cardiology Program will conduct the personalized risk assessments and provide individual education. All test results will remain confidential. Referrals for intervention are made when necessary.

STRESS AND HEART HEALTH
Stress may trigger a heart attack if you are at risk. It may also cause you to adversely alter your eating, drinking and exercise habits, thus increasing your risk of heart disease. The NewYork-Presbyterian Hospital Preventive Preventive Cardiology Program can set up a screening service at your corporation that will help identify employees who are at risk for heart attack and educate them about risk reduction.

NewYork-Presbyterian Hospital Preventive Cardiology Program
is a comprehensive program that supports patient care, education, research and community and corporate outreach to reduce the burden of cardiovascular disease and to promote heart health. A highly trained staff of doctors, nurses, health educators and others under the direction of Lori Mosca, M.D.,Ph.D. established the Passport to Heart Health Program as a service to New Yorkers and to establish a national model for preventive care.
